Eligibility
Youth and young adults eligible for PATHS are:

  • Between the ages of 18-21
  • Intersected with child welfare or youth justice
  • Without or at risk of being without housing

Referrals
Complete our Referral Form to refer a young adult to PATHS. Referrals received after October 1st, 2026, will be forwarded to our Housing Resource Line to receive support with resource navigation, case management, and Coordinated Entry.

CW Solutions operates one of two PATH programs in Wisconsin (the other is operated in Milwaukee by Pathfinders). PATHS is a program for emerging adults, age 18-21 who had some type of child welfare interaction while they were a minor who now need housing support. 

PATHS utilizes Housing First and Rapid Re-Housing frameworks to provide immediate housing supports and wraparound services to facilitate long-term housing stability for youth. Our program’s focus is to empower youth to work on their goals in  five core service areas:

  1. Permanent Connections
  2. Academics
  3. Training and Employment
  4. Housing
  5. Social and Emotional Well-being
