We are recruiting for a split role of Comprehensive Community Services (CCS) Service Provider and Family Keys Housing Navigator. This is position provides services in Wood County, Wisconsin and generally works afternoons and evenings (in a range between 11am - 8pm) during the work week (Monday - Friday), with the schedule flexible based upon the needs of the individuals served.
For CCS:
According to the Wisconsin Department of Health Services (DHS), “CCS is a program that helps individuals of all ages live their best life by providing support that addresses their unique needs related to mental health and substance use. CCS is intended to assist individuals who are in need of care outside of inpatient settings, but who may have ongoing needs that if left unaddressed, could result in hospitalization during times of crisis.”
CW Solutions' CCS Mental Health Service Providers serve diverse clients by providing mental health management services to individuals, their families and natural supports in both individual and group settings. To be effective, the skill development specialist works with individuals in their homes and communities.
The worker is responsible for mentorship and technique-building, working collaboratively with the CCS wrap-around team to assist individuals in developing skills to successfully manage their mental health diagnosis while engaging with their family, school, employment and social life.
This position requires flexibility of schedule in response to the needs of clients; hours will primarily occur during late afternoon and early evening hours.
For Family Keys:
The Housing Navigator provides a system of care that is person-centered with the needs of the family dictating the types of services provided, with an emphasis on gaining and sustaining permanent housing stability. To be effective, the Housing Navigator will work with families in their homes and communities, incorporating their immediate support systems to provide wraparound services.
This position performs ongoing, supportive case management services to families. This position is responsible for technique-building and Family Systems case management, working collaboratively to develop a Strengths-Based prevention plan that assists families in increasing resiliency, positive emotional vulnerability, and community building.
This position is family-driven and focuses on the family using their voice to drive their plan. The position must develop family’s strengths, promote positive relationships and connections, and provide the support and follow-up services needed to address the range of challenges they confront to sustain safe and stable housing.
The following are the essential job functions for CCS Service Providers. Functions are always customized to each Individual Recovery Plan.
Primary Functions
- Provide individual skill and psychoeducation services through modeling, cuing, educating, social and emotional support, and other methods that best match the individuals needs and learning style to address their mental health diagnosis and needs.
- Set realistic expectations and goals based on each individual’s strengths, talents, abilities and needs.
- Participate in regular CCS team meetings.
- Provide regular communication and feedback to the assigned CCS Facilitator.
- Maintain accurate and detailed accounts of all CCS activities in the designated CCS electronic health care record system as well as in the employee’s time and expense sheets. Entries must be completed daily, no later than one business day after any action or interaction that requires documentation.
- Collaborate with other community partners and resources for goal-related activities under the Individual Recovery Plan.
Additional Functions
- Provide support and back-up services for CCS clients assigned to other CW Solutions CCS providers.
- Cross-train in LEO brain science based programming and the Adolescent Diversion Program tools.
- Apply trauma-informed care and practice inclusion and allyship in all activities and services.
- Complete required initial and annual training necessary for CCS certification.
The following are the essential job functions for a Family Keys Housing Navigator.
Primary Functions
- Provide holistic and comprehensive services to all participants including intake, assessment, goal setting, progress monitoring, advocacy and referral.
- Utilize the Housing First model and philosophy in all aspects of programming, including ensuring every eligible served participant has safe housing, regardless of the reason or actions that resulted in homelessness or risk of homelessness.
- Assist the participant in identifying their expectations and goals, based on their wishes, strengths, talents, abilities and needs. Develop and maintain a written action plan based upon their identified goals and needs.
- Maintain accurate and detailed accounts of all participation in the program using internal tracking systems.
- Develop and sustain integrated relationships with housing programs, community resources and other agencies for community building and family support. Work collaboratively with providers to provide successful outcomes for participants.
- Expand housing opportunities for participants by developing relationships with landlords in communities throughout Wood County.
- Maintain the Housing Index and other tools to track details of housing supports and the working relationships with landlords and other community partnerships.
- Coordinate with Family Services and FSET within Wood County Human Services Department for shared case management services.
- Assist participants with co-enrollment in other supportive programs, including FSET, WIOA, W2, and DVR in achieving their educational and employment goals.
- Teach independent living skills, in collaboration with Family Services, with an emphasis on housing skills including effective housing search, understanding leases, tenant rights and landlord rights.
- Help facilitate participant voice and feedback with program development, policies and services including regular listening sessions and other methods established in the program.
- Participate in North Central Continuum of Care including Coordinated Entry and Point-In-Time.
- Attend required state and agency training.
Education: Required - A bachelor’s degree in Social Work, Sociology, Counseling, Human Services, Counseling, AODA, Health and Wellness or related field that provides the necessary skills to meet the expectations of the position.
Experience: Required -
- Proven background experience in working with individuals who express a heightened trauma response and experience mental health issues.
- Proven experience and knowledge of intervention services and crisis management.
- Knowledge of human behavior, motivation, crisis intervention, individual, family and group dynamics and organizational skills are necessary.
Transportation: Required - This position requires the staff member hold a valid driver’s license and use their personal vehicle. Mileage reimbursement is provided for all business-related travel.

- Basic everyday living skills, the ability to understand and follow directions (agency manuals, etc.), and reading and writing (for reports) is necessary.
